Who would I contact about my landlord not fixing the black mold growing in my house im renting?

Im renting from this lady and ever since I moved in I have had nothing but problems.. The lease was signed by her but come to find out I was actually renting it from a man which she signed his name. Then she changed things on the lease after I signed it. She charged me an outrageous deposit and the house was so dirty it took me two days to clean and It was also full of the previous tenants stuff. The ceiling in the hallway had a steady leak for the first two weeks. It turned out to be from the air conditioner which had damaged the ceiling so bad that it began to fall through. After we fixed the leak and checked it out some more we noticed that there was black mold everywhere in the ceiling and in the attic. The landlord gave me $20 to buy drywall to fix where it was falling through. The landlord kept telling me that she would hire someone to remove the black mold and it has been months now and the mold is still there. I finally found somewhere else to go so I am moving out tomorrow because I cannot breath at night due to the large amount of mold. It blows through the vents throughout the house and has made breathing very difficult for everyone that lives here. She knew that we had nowhere else to go and wouldn't fix the problem so she made us suffer. I called everyone from code enforcement to the health department and nobody could help me. She is now charging me for breaking my lease. I do not know who to call about my landlord renting out this house with the black mold growing in it. I have pictures, written statements, receipts from where I tried to fix it, witnesses and copies of my request to have it fixed. I have done everything carefully as I was supposed to from the day I moved in. I have kept doentations of everything from day one, even the illegally signed lease she tried to forge. Does anyone know who I should contact about this property? I don't want someone else to fall into her trap and get sick from black mold and also get robbed out of their deposit! Does anybody have any answers besides getting a lawyer and filing a lawsuit? Shouldn't the health inspector or someone be able to stop her?
