Do I need a hysterectomy?

I'm 37 and about 1.5 years ago I started having extreme fatigue and very heavy and painful periods. My doc put me on on oral provera, 10 days on and 20 off. The provera didn't help any of my symptoms and gave me oral thrush which I still can't get rid of. I went off the provera and was put on 850 mg. metformin. It did nothing. My periods are getting more painful every month. The bleeding is still very heavy for 4 out of 8 days that I bleed. An ultrasound and endometrial biopsy showed a small ovarian cyst that went away on it own, but nothing much more. I don't have pain during , just during my period. My doc has now put me on ortho micronor birth control pills to help with the heavy bleeding. I started taking them 11 days ago. My period flow is slightly lighter this time, but the pain of it is much worse. The ortho micronor also has made my legs and hands swell up with water retention. I take 800 mg. of ibuprofen 2 or 3x's a day for period pain. I'm getting sick of all the meds and hle of this. What is wrong with me? Do I need a hysterectomy or is there a med out there that will actually work to ease this? I'm willing to try anything. I can't take much more of this.
